Creative Portfolio - The creative portfolio is a compilation of your original creative work. It can include sketches, illustrations, photos, music, or literary work — any proof of creativity.

Updated Medical Records signed by your Physician in their Official Letterhead

Steps

STEP 1:

Submit all the listed requirements via email to tinabaron@sofa.edu.ph only in JPG or PDF format.

Subject of email must be: COMPLETE APPLICATION DOCUMENTS FOR SoFA

1.) Completed Registration Form

2.) 1 colored photo with a white background

3.) 1 official/government issued ID

4.) Creative Portfolio - The creative portfolio is a compilation of your original creative work. It can include sketches, illustrations, photos, music, or literary work — any proof of creativity.

5.) Copy of your NSO birth certificate

6.) Updated Medical Records signed by your Physician in their Official Letterhead


The following must be sent to SoFA at roughly the same time as the list above.

1.) Certificate of Good Moral Character. Applicable to high school graduates and college transferees. Documents must be sent directly from the school to the SoFA via email to tinabaron@sofa.edu.ph. Any template will be accepted.

2.) Character Reference Letter. Applicable to college graduates. The Letter must be sent directly from the person issuing the letter to the SoFA via email to tinabaron@sofa.edu.ph. Any template will be accepted.

3.) Form 137 & 138 or other forms of Grade Assessment. Applicable to recent high school graduates. Form must be sent directly by your school to SoFA via email to tinabaron@sofa.edu.ph

4.) Transfer Credential Certificate and Transcript of Records. Applicable to college transferees. Form must be sent directly by your school to SoFA via email to tinabaron@sofa.edu.ph

5.) Transcript of Records. Applicable to all college graduates. Form must be sent directly by your school to SoFA via email to tinabaron@sofa.edu.ph


STEP 2:

Upon submission of all the requirements, the application documents will then be evaluated by the Admissions Team and a DESIGN TEST will be sent to you, via email, for completion within 24 hours upon receipt of the same. Please send the completed test via email to tinabaron@sofa.edu.ph, Director of Admissions.

Subject of email must be: DESIGN TEST of (your complete name)


STEP 3:

The Director of Admissions will schedule an Admissions Chat with you 2 - 3 working days after the receipt of your test. The Admissions Chat will focus on your design potential, social maturity, and simply getting to know who you are and how you view the world.


STEP 4:

Please wait a minimum of 10 working days to receive a decision letter. If you are accepted to SoFA, you must pay the non-fundable PHP 10,000 reservation fee 10 banking days following the date on the acceptance letter.
